Overview:
We are seeking a compassionate and skilled Registered Nurse to join our Home Health team. As a Registered Nurse, you will have the opportunity to provide high-quality care to patients in the comfort of their own homes. Responsibilities:
-Conduct physical examinations and assessments of patients
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed
- Monitor and evaluate patient progress and adjust care plans accordingly
- Provide education and support to patients and their families
- Collaborate with interdisciplinary healthcare team members
- Maintain accurate and up-to-date medical documentation
- Perform phlebotomy and IV insertion as needed

Skills:

- Proficiency in medical documentation systems, such as Athenahealth
- Experience working in a nursing home or senior care setting is preferred
- Ability to lift and transfer patients safely
- Excellent communication and interpersonal skills
- Attention to detail and strong organizational abilities
